R.N Care Coordinator

FullTime

Location: Community Sites: Gloucester and Camden Counties

VITALity Catholic Healthcare Services is looking for a registered nurse with a vocation to serve seniors. In this unique position you will help connect seniors and persons with disabilities to a variety of medical social spiritual and community resources and services.

Responsibilities: The Care Coordinator will be responsible for coordinating care for Seniors and Persons with Disabilities in the Diocesan VITALity Catholic Healthcare Services Program. The Care Coordinator will establish effective relationships with patients families professionals payers and other relevant parties; Conduct face to face and telephone assessments on the persons physical social psychological financial and caregiver status as appropriate to identify the persons strengths and limitations related to the identified concern. The Care Coordinator will develop individualized care/service plans and access services if indicated. Teaching skills and goal setting by the coordinator is essential to help individuals and caregivers achieve positive outcomes.

Requirements: Registered Nurse (RN) with current license in New Jersey. Minimum 3 years of experience in health care preferably community setting care coordination discharge planning home care or case management. Highly organized and ability to work autonomously. Demonstrated skills in assessment leadership communication counseling/consultation problem solving and teaching. Bilingual in English/Spanish is a plus.
