Reports to: Nurse Manager
Work Model: OnSite
Wage Range: $44.51 to $55.64 per hour
Hours: 37.5 hours per week
Shifts: Days 8 10 or 12 hour variations with occasional weekend and evening
Status: Permanent FullTime
Closing Date: May 15 2025

Position Summary
 

In the Toronto Western 38 bed Medical Surgical Neuro ICU (MSNICU) the interdisciplinary team provides care to patients who require advanced ventilatory and hemodynamic support to overcome the complex challenges of neurosurgical emergencies shock syndrome and multisystem failure.

Reporting to the Unit Manager as a member of the multidisciplinary team on the MSNICU the Patient Care Coordinator acts as a resource for the multidisciplinary team. This position plays a leadership role by mentoring and providing direction ongoing development and preceptorship to team members.
 

Furthermore as a member of the MSNICU you will have a variety of career development opportunities such as:

  • Involvement in Quality Improvement and/or research projects as part of the Krembil Nursing Awards
  • Nursing fellowship opportunities through the Collaborative Academic Practice within UHN
  • Involvement in unitbased councils to implement change

Duties

Patient Care Coordinator (PCC) Responsibilities/Duties 

  • performing human resources financial and administrative responsibilities to ensure efficient and effective unit operation
  • liaising with manager and P&C team members for recruitment of new employees including but not limited to: screening applicants booking interviews participating in interviews reviewing references and liaising with potential candidates
  • Supporting administrative onboarding process such as registering candidates for eLearning or coordination with APNE for onboarding processes.
  • liaising with members of the multidisciplinary team to ensure high standards of quality and optimal management of patient care outcomes
  • contributing to nurses students and ancillary staff development by promoting and ensuring a supportive and collaborative learning environment within the service(s)/cluster area based on the principles of adult learning and practice standards
  • performing crossfunctional duties consistent with the job classification as assigned or requested
  • The PCCs other responsibilities include supporting the Colead for the Critical Care Response Team (CCRT) working with the physician lead and members of the CCRT to meet the objectives of the team.

Qualifications :

  • Baccalaureate in Nursing required Masters preferred/prepared
  • Three years nursing experience in a tertiary care ICU
  • Current MSNICU experience preferred
  • Previous leadership experience preferred
  • CCRN an asset
  • Demonstrated leadership abilities required with prior leadership experience preferred
  • Demonstrated ability towards team building and collaboration
  • Effective organizational interpersonal and communication skills
  • Client service oriented with the ability to effectively work with diversity and appreciate that people with different opinions backgrounds and characteristics bring richness to the challenge or situation at hand
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
  • Exercise initiative and good judgment with ability to multitask
  • Excellent decision making and problemsolving skills
  • Clinical and patient teaching skills
  • Excellent time management skills
  • Excellent skills in MS Word Excel SharePoint EPIC and Incident Reporting software
