Job Description
We are seeking a Care Managment Coordinator to join our team on a Casual basis. The Care Management Coordinator is a member of an interdisciplinary team accountable and responsible for coordinating the care and service of selected patients and for the promotion of collaborative practice and continuity of care. As a result effective utilization of resources and the achievement of desired clinical and financial outcomes will be accomplished. Will monitor the appropriateness of hospital admissions and extended hospital stays.
MINIMUM QUALIFICATIONS:
- RN with an Associates degree from an accredited College.
- Minimum five years experience as a Registered Nurse in a clinical setting performing utilization review case management care planning and quality improvement
- Current State of Wisconsin Registered Nursing license
Preferred Qualifications:
- RN with a Bachelors degree from an accredited College or University.
