Position at Nuvance Health Med Practice CT
Affiliated with Danbury Hospital Nuvance Health Medical Group is seeking an experienced fulltime Geriatric Nurse Practitioner (APRN for SNF and ALF access in the Danbury area. Most of the time will be spent at Meadow Ridge as your home base in Redding CT with time needed at our other locations in Bethel Danbury Newtown Southbury and Wilton. This newly created role includes working to provide primary care access for Meadow Ridge in the Skilled Nursing Facility the Assisted Living Facility and in the Primary care office located on site in Redding CT.
Meadow Ridge is a luxury continuing care retirement community in Redding CT offering Independent Living Assisted Living Memory Care and Skilled Nursing. The position will be onsite MonFri. Hours can be tailored to best support a worklife balance while still meeting the needs of the residents. A threemonth mentorship program is part of your initial training experience.
Job Responsibilities:
The APRN provides medical care for patients in collaboration with a physician assisting patients to recover from acute illnesses while minimizing the effects of chronic illnesses.
Job Requirements:
- This position requires a minimum formal education or masters degree NP State of CT RN and APRN license CT Controlled Substances and Federal DEA Registration and BoardCertified Nurse Practitioner. AGNP preferred but willing to consider FNP candidates with geriatric experience.
- At least one year experience as an APRN in Primary Care/Geriatrics is required.
