Job Summary:
The Resident Services Coordinator provides services to a diverse group of residents throughout CCDCs housing portfolio. This position works within a traumainformed care model utilizing best practices and recognizes the impact of the social environment and the importance of community to improve an individuals quality of life. This position is responsible for planning and coordinating ongoing onsite programming including health and wellness aginginplace education cultural events community organizing and community building and engagement; provides information and referral helping residents in need of higher care coordination including APS and clinical support; utilizes clinical techniques such as deescalation and motivational interviewing; develops new relationships and partnerships with outside service providers to bring new ideas resources to our residents; and supports residents around housing stabilization needs. This is a floater position where the person provides services at multiple sites including 50 at 1005 Powell and 50 at other properties as assigned.Status:Fulltime Nonexempt

Hours:Monday through Friday 9:00 a.m. to 5:30 p.m. plus some nights and weekends

What You Will Do:- Complete initial and ongoing outreach to residents inviting them to access services as needed including light case management information and referral housing stability assistance coordination with Property Management and other third party providers
- Provide support across the Resident Services team and the diverse residents residing in CCDCs housing portfolio as assigned by a referral system; both sitebased placement (up to three months) and onetime (an event or outreach) support can be provided.
- Provide residentcentered direct services such as general information referrals translation/interpretation assistance and service connections to local social service and government agencies
- Collaborate with Resident Services team members to create implement lead and evaluate programming/activities/events at the properties including a focus on aginginplace health educational workshops and resident empowerment
- Collaborate with onsite staff other CCDC departments and city funders
- Organize setup implement and evaluate activities including larger community events focusing on crosscultural community building
- Build maintain and monitor relationships with social service agencies that focus on aging and health to bring additional programming to the property
- Support residents to access clinical support including referring to the Resident Services Wellness Team or Adult Protective Services
- Work on other resident services projects such as an InterProperty field trip food programs (Meals on Wheels food box program food drives food pantries etc. holiday events and greening activities
- Support resident leadership and empowerment through volunteerism and Educate and/or link residents to community/neighborhood projects that impact their quality of life so they may provide input
- Complete administrative functions such as case notes intake and data entry in RS database and other paperwork as needed

Education and Experience (Minimum Qualifications):- Bachelors Degree in Social Work or related field and two 2 years of experience in providing direct services in the form of information referrals and/or linkages to community and public resources to lowincome populations and one 1 year of experience in developing organizing and implementing group activities or
- High school diploma or GED and a combination of five 5 years of experience in providing direct services in the form of information referrals and/or linkages to community and public resources to lowincome populations as well as experience in developing organizing and implementing group activities
- Bilingual and biliterate in English/Chinese
- Experience working with older adults
- Experience working with adults with mental health and or substance abuse issues
- Knowledge of and sensitivity to the needs of lowincome population
- Ability to travel and coordinate services for several buildings in San Francisco
- Demonstrated knowledge and commitment to community organizing as a tool to increase resident engagement and leadership development
Preferred Qualifications:- Knowledge of local resources and public benefits assistance
- Experience providing services to older adults and adults with substance use/mental health needs
- Experience delivering services to SRO resident communities
- Experience working with onsite Property Management to promote community wellness
