Gilead Community Services is seeking an attentive and recoveryoriented Case Manager to join our multidisciplinary residential/supported apartment program staff.
About the position:
The Case Manager for Gileads Clinton area adult residential/supported apartment program works SuTh 7a3pproviding case management advocacy educational and supportive services in a program serving adults with major mental illness and cooccurring substance use disorders. The individual in this role carries out all phases of clinical services including intake engagement assessment treatment planning case management referral and discharge. This role also requires the timely completion of required documentation including assessments and other treatment planning.

This is a union position and pays $18.8920.69 per hour dependent on level of education. The compensation also includes a $1500/year stipend added to the base pay over 26 pay periods.
Responsibilities:
