The Multnomah County Health Departments Primary Patient Care team is looking for a Licensed Practical Nurse who deeply desires to make a difference in their community.

The Licensed Practical Nurse works within the health care team and provides focused community and clinic health services directed towards the prevention of disease the promotion of health the reduction of disability improving the clients selfcare using professional judgment in focused/limited assessments screening contributing to care planning intervention and provides health information/education to patients. The Panel Manager role tracks the health of the population of patients assigned to the provider panel. Manages team communication including phone messages fax requests and warm handoffs as needed. The Panel Manager is responsible for the team InBasket within the electronic health record system and delegates requests to appropriate staff in addition to completing those requests that are within the scope of a LPN. The Panel Manager completes needed patient visits as noted on EPIC team schedule in addition to providing focused assessments to walkin clients. Supports providers when staffing indicates the need. The position provides services in many languages and utilizes interpreters via phone tablet or inperson.
As a Licensed Practical Nurse you will provide :
Panel Management
TechSavvy Efficiency: Leverages electronic reports and scheduling systems to proactively identify and address patient needs for preventive screenings ensuring timely care and improved health outcomes.
PatientCentric Preparation: Proactively conducts previsit activities such as chart reviews and team huddles ensuring each patient encounter is wellprepared and personalized for optimal care.
Collaborative Chronic Disease Management: Partners with providers and the care team to actively monitor chronic disease and prevention reports initiating timely followups or referrals to optimize patient health and wellbeing.
Team/Client Communication
Communication Powerhouse: Seamlessly manages multiple communication channels (phone email inperson) ensuring clear and effective information flow across the care team and with patients in any language utilizing interpreters if needed..
Organization & Efficiency Expert: Masters both electronic and paper systems expertly handling inboxes scheduling and followups to keep the team running smoothly and patients wellinformed.
PatientCentric Advocate: Proactively participates in reminder calls follows up on noshows and ensures patients receive critical information about referrals treatments and medications.
DetailOriented Documenter: Maintains accurate and comprehensive patient records utilizing SOAP notes EPIC phrases and clear communication to support highquality care and collaboration.

Dynamic Clinical Support: Provides comprehensive clinical assistance to providers performing a wide range of tasks from patient intake and vitals to administering medications and assisting with procedures.

Skilled Multitasker: Juggles diverse responsibilities seamlessly from performing diagnostic tests and administering treatments to completing documentation and assisting with referrals.

To Qualify:
We will consider any combination of relevant work experience volunteering education and transferable skills as qualifying unless an item or section is labeled required. Minimum Qualifications/Transferable Skills*:
An Associates degree or a Bachelors degree in nursing from an accredited nursing program
Possession of or ability to obtain an appropriate and valid license to practice as a Licensed Practical Nurse issued by the State of Oregon by the time of hire
A minimum one 1 year of nursing experience
BLS/CPR certification required
