Job Title: Peer Specialist

Purpose of Position:
The Peer Specialist is an individual who has lived experience with substance use and/or mental health conditions. This position actively supports Sponsors participants who are returning to the community postincarceration. The goal of a Peer Wellness Specialist is to help participants establish health wellness and recovery support systems within the broader community.
Position Reporting:
This position reports to the Health & Wellness Director.
This position does not have supervisory responsibilities.
Job Responsibilities:
Connect participants to health wellness and recovery support systems in the broader community.
- Facilitate the Health & Wellness portion of New Participant Orientation to inform new participants of health wellness and recovery services and support at Sponsors.
- Manage participant referrals. Contact newly referred participants within 48 business hours to ensure timely care.
- Meet with participants and help them identify their health wellness and recovery support system needs. These often include:
Primary Care Provider
Specialists
Therapist
Psychiatrist
Dentist
Optometrist
MAT Provider
Support Groups
Long Term Peer Support
Disabilities Services
Crisis Services
- Ensure participants have active health insurance.
- Verify OHP eligibility and enroll participants if needed.
- Help participants schedule care team appointments. Provide transportation and support at appointments as requested.
- Provide ongoing support to help participants meet their health wellness and recovery goals.
- Assist participants in identifying personal goals including strengths and growth opportunities involving recovery health and overall reentry concerns.
Demonstrate healthy selfcare
- Rolemodel healthy boundaries and positive selfcare
- Discuss prosocial thinking and behavior
- Provide individual daytoday coaching with participants
- facilitate support groups or programs that support participant wellbeing
Participate in collaborative care team meetings to support participants in program completion
- Contribute to the development of case plans intervention strategies and discharge planning
- Cultivate professional relationships with community partners
- Coordinate appropriate case management referrals
Maintain accurate records and communication
- Maintain thorough participant files and case notes
- Utilizes Sponsors log case management system and other electronic/physical communication tools
- Maintain timely communication with other members of the care team program partners and other Sponsors staff
Other duties as assigned
- Perform other related duties as assigned by the Health & Wellness Director
Skills and Knowledge:
- Demonstrated understanding of the reentry process
- Knowledge of mental health substance use and health impacts of incarceration.
- Knowledge of health wellness and recovery community resources.
- Ablility to work independently with little supervision
- Willingness to work some nights/weekends and holidays
- Ability to demonstrate and teach basic problem solving
- Excellent understanding of professional boundaries and appropriate scope of practice in a participantcentered work environment.
Education and Experience:
- Minimum of 1 year experience providing peertopeer support services
- Experience as a Certified Peer Wellness Specialist or Community Health Worker is preferred
- Experience providing healthcare navigation is preferred
Work Environment:
- Peer Wellness Specialists work in the field meeting participants offsite when necessary. Work will be performed in a variety of settings including Sponsors campuses recreational sites and groups communitybased outpatient settings community corrections agencies etc.
- Use of personal vehicle may be required.
- The incumbent frequently works in an office environment and uses a computer telephone and other office equipment as needed to perform duties.
- May encounter frequent interruptions throughout the workday.
- May interact with individuals experiencing active addiction or mental health crises..
Physical Demands:
- Walking up and down stairs
- Sitting at a desk for prolonged periods of time
- Noise level in the work environment is typical of that of an office environment.
Postconviction Supervision Status :
Successful candidates for this position must not be on parole or probation and cannot be in a diversion program (e.g. SB416 Treatment Court DUII Diversion Restorative Justice).
Employment is contingent on a background clearance by the Oregon Youth Authority through their external review and approval process.
