The PreAftercare Coordinator will oversee all PreAftercare Referrals for participants within YSS Aftercare Coverage Areas 42 counties) including Woodward Academy and the State Training School. The position is responsible for all PreAftercare direct services provided at Woodward Academy in Dallas County. The PreAftercare Coordinator will also play a crucial role in the transition planning process partnering with various professionals within HHS JCS and other agencies.
This role involves managing PreAftercare referrals within YSS Aftercare Coverage Areas ensuring smooth transitions for participants by assigning referrals providing case management and maintaining a presence at Woodward Academy and the State Training School. Responsibilities include attending Youth Centered Planning Meetings fostering collaboration between agencies and staying informed on program policies to ensure compliance. The position requires completing accurate monthly billing training professionals on referral processes and presenting services to promote awareness and community outreach. Additionally the role involves developing partnerships with HHS Juvenile Court Services and other community resources as well as participating in relevant meetings to support program goals.
Qualifications
REQUIRED EDUCATION KNOWLEDGE EXPERIENCE/SKILL
Bachelors Degree required Human Services or a related field
Three years of experience in direct human services work with highrisk youth or young adults
one of which must be case management is required.
A strong background in working with highrisk youth
Case Management experience is a priority and preferred
Leadership skills and experience is required
Computer Literacy (Word Excel Access Outlook Internet)
Proven ability to multitask with close attention to detail
Ability to coordinate and collaborate effectively with others
Ability to convey information effectively through speech and writing skills
Critical thinking problem solving and time management skills are necessary
