Job Title:Social Worker Rotherham Hospital Discharge
Location: Rotherham
Pay Rate: 33 Per Hour
Liquid Personnel is recruiting a Qualified Social Worker for the Adults First Contact Team at Rotherham NHS Foundation Trust. To be successful in this role you must have Social Work England registration be eligible to work in the UK hold a qualification in Social Work at degree level and have postqualified experience.
What will your responsibilities be:
The role involves completing Care Act assessments on wards supporting MDTs organizing and prioritizing caseloads for timely discharges conducting Mental Capacity Assessments Best Interest Discussions and safeguarding adults. Experience in fastpaced environments and knowledge of the Mental Health Act are essential.
