The Adult Outpatient Therapist plays an intricate part in the success of our clients in need of support services. This position meets individuals on a regular basis to ensure the treatment plans and established care plans are being followed. They will assess needs and behaviors do well checks and be the advocates of the clients medical and personal needs. Defining their progress and reactions of care to the therapeutic team if needed so plans can be adjusted.

- Clinical Licensure (LPC LICSW or LMFT)
- Excellent Oral and Written Communication Skills
- Proficient in typing computer use and email
- Good organizational and documentation skills
- Ability to communicate effectively
- DSM Knowledge; Ability to type
- Knowledge of community agencies
What youll be doing:
- Conduct comprehensive individual assessments on new or returning adult clients
- Conduct individual family and group therapy sessions
- Assess clients risk of suicidal/homicidal ideations
- Maintain confidentiality of client information treatment and records
- Perform timely documentation of the assessment in EMR
- Develop a treatment plan based on clinical experience and knowledge in partnership with the client
- Present cases to psychiatrists regarding clients medication and/or need
- Provide crisis intervention as needed and/or required
- Collaborate with internal staff regarding clients progress such as Case Managers Medical Staff etc.
- Act as a client advocate to coordinate required services or to resolve emergency problems in crisis situations
- When necessary referral to internal or external specialists or community resources to assist the client in treatment options
- When applicable assist family members in understanding dealing with or supporting clients
- Participate in various meetings to include departmental divisional or other meetings as required
- Maintain current on new developments in counseling
- Complete billing documentation in a timely and accurate manner
- Conduct follow up with clients internal and/or external individuals
- Maintain Licensure
- Locate identify and build relationships with unsheltered individuals
- Provide immediate support intervention and connections with community programs
- Facilitate therapy as a primary intervention
- Utilize community resources
- Attend Continuum of Care meeting as scheduled
- Provide crisis intervention as necessary
- Maintain knowledge of community resources
- Effectively communicate with treatment team
- Maintain uptodate records in HMIS and Avatar
- Complete other duties as assigned by Manager
