An exciting job role has recently become available at the Stockport Metropolitan Borough Council. They are looking for a new member for their Adult/Hospital Discharge Team.
What will your responsibilities be
In this role youll collaborate with Pennine Care colleagues to enhance mental health services and outcomes for individuals transitioning out of the hospital. Your responsibilities will include ensuring safe and timely discharges for those with functional or organic mental health conditions working closely with individuals carers health professionals advocates and thirdsector colleagues. Youll coproduce Care Act assessments support plans and risk assessments conduct Mental Capacity Assessments and chair Best Interests Meetings. Additionally youll contribute to improving the discharge process through effective teamwork and partnership.

Qualifications and Experiences:
To be eligible for this role you must have Social Work England registration and be eligible to work in the UK. Additionally you need to hold a full UK driving licence and possess postqualified experience. A degree or equivalent qualification in Social Work is also required.
