Rate: 35 per hour
The Cheshire West and Chester Council islooking for a Social Worker to join their Hospital Discharge team offering a pay rate of 35 per hour.
What will your responsibilities be
In this role you will assess the need for social work services identifying risks and the need for protection determining intervention methods and advising on the use of appropriate social services and resources. You will liaise and negotiate with other professionals statutory and voluntary agencies to ensure the best possible service for clients. You will manage and monitor your allocated workload within individual and group priorities and County policies complying with statutory obligations. You will maintain appropriate records of your work and carry out required administrative procedures. Additionally you will prepare for and attend supervision sessions and staff meetings making use of all available training and development opportunities. You will contribute to the evaluation and development of services by sharing knowledge about theory skills and practice with other social services staff professional groups and interested bodies. Where appropriate you will also supervise students.
