Position Overview:
The Hospice and Palliative Care Nurse Practitioner PartTime role plays a vital role in delivering compassionate and specialized medical care to patients facing lifelimiting illnesses along with providing essential support to their families during the challenging endoflife journey. This role demands a unique blend of clinical expertise empathy and communication skills to ensure the highest quality of care and comfort for patients in their final stages of life.
Minimum Qualifications:
1. Masters or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
2. Current and unrestricted Nurse Practitioner license in the state of Washington.
3. Current Washington State license as an Advanced Registered Nurse Practitioner (ARNP) or Family Nurse Practitioner (FNP).
4. Strong clinical skills including proficiency in pain management and symptom control.
5. Excellent communication and interpersonal skills with the ability to provide compassionate care to patients and families.
6. This position is designated as Safety Sensitive. Must be able to pass a preemployment drug screen which includes cannabis testing in Washington State.
7. Available to work evenings weekends and holidays.
Preferred Qualifications:
1. Bilingual in Spanish
2. Certification in Hospice and Palliative Care.

RESPONSIBILITIES AND ESSENTIAL FUNCTIONS
Patient Assessment and Care Planning under the supervision of the Medical Director: Conduct thorough patient assessments to determine their physical emotional and psychosocial needs.
Collaborate with the interdisciplinary team to develop personalized care plans that focus on pain management symptom control and quality of life enhancement.
Regularly evaluate and adjust care plans based on patient response and changing conditions.
Symptom Management and Pain Control: Provide expert symptom management including pain control nausea management and other distressing symptoms associated with the patients condition.
Prescribe and administer medications as necessary to alleviate discomfort and improve the patients overall wellbeing.
Monitor medication efficacy and side effects and adjust dosages as needed to optimize patient comfort.
Patient and Family Support: Offer compassionate emotional support to patients and their families addressing fears anxieties and concerns related to endoflife care.
Facilitate open communication between patients and their loved ones ensuring their wishes and preferences are respected and honored.
Communicate effectively and respond appropriately to patients and families in both routine and crisis situations.
Care Coordination: Collaborate with other healthcare professionals including physicians nurses social workers chaplains other healthcare and administrative professionals to ensure a holistic approach to patient care.
Coordinate care transitions such as admissions discharges and transfers to ensure a seamless and supportive experience for patients and their families.
Documentation and Compliance: Maintain accurate and uptodate medical records including assessments care plans progress notes and other relevant documentation.
Adhere to all legal and regulatory requirements related to palliative and hospice care ensuring compliance with state and federal guidelines.
Education and Support: Provide education and training to healthcare staff patients and families on pain management symptom control and endoflife care options.
Conduct educational sessions and workshops within the community to raise awareness about hospice and palliative care services.
Community Outreach: Represent the agency at meetings conferences and community events.

Compensation
Hiring Range: $61.06 $78.28 per hour
Position Pay Range : $55.38 $85.84 per hour
Premiums:
$15.00/hour On Call/Standby
$2.25/hour Weekend day
$4.75/hour Weekend Night
$2.50/hour Weeknight Premium
Note regarding the listed pay range:
Hiring Range: The starting pay range offered to new hires depending on experience and qualifications.
Position Pay Range/Salary Range: The full pay range for this position from minimum to maximum based on role responsibilities and market rates.
At TriCities Chaplaincy we carefully establish pay ranges for our positions by taking various factors into account including education certifications knowledge skills experience and market conditions. These ranges are tailored to match the diverse programs and services we offer. Our goal is to ensure fair compensation for candidates based on these factors while maintaining internal peer equity.
Benefits
Mileage Reimbursement: $0.70 per mile for eligible positions.

AGENCY REQUIREMENTS
1. Demonstrate support and adherence to the agencys Mission Vision and Values.
2. Participate in community Education and Outreach to raise awareness of agencyprovided programs including but not limited to hospice care advanced care planning and bereavement.
3. Responsible for knowledge and compliance with agency policies procedures and practices.
4. Responsible for maintaining and protecting Patient and agency information in a confidential manner. Practice appropriate HIPAA confidentiality by accessing patientrelated information on a need to know basis only.
5. Reliable personal transportation and current and valid drivers license and vehicle insurance.
6. Satisfaction of state and agency requirements for Tuberculin screening.
7. Hepatitis B immunization series proof of series or signed declination.
8. Proof of applicable Vaccination(s) on file or approved accommodation/declination as appropriate on file.
9. Must pass a criminal background screening and preemployment drug screening.
10. Ability to meet the physical demands of the position (see attached physical requirements and environmental conditions).
11. Complete agency and departmental orientation and training program(s).
12. Complete all continued learning education policy reviews and required training by their respective deadlines.
13. Maintain licensure(s) and certification(s) required for the position.
14. Available to work as assigned/scheduled.
