Job Title: Physical Therapist PT / DPT
Compensation: $50.00 $120.00 per visit

Position Summary: Home Health Care Inc. is seeking Physical Therapistslooking for autonomy and variety to join our therapy team and help clientsthroughout the Twin Cities Metro area! Design functional treatment plans forclients in their home environments building interpersonal connections withthose you care for by helping clients achieve their goals to remain healthy athome.
Responsibilities:
- Provide Physical Therapy / PT services to our clients in their homes
- Admissions / SOC (Starts of Care) evaluations initial and ongoing assessments
- Develop and revise patients plan of care in collaboration with PTAs and other clinicians
- Plan evaluate and treat assigned clients then evaluate outcomes
Qualifications:
- Possess a current PT / Physical Therapy license to practice in the State of Minnesota
- Experience in home care setting namely OASIS documentation
- Demonstrated ability to work autonomously
- Valid Drivers License and reliable transportation
Military Occupational Classification (MOC) Codes: If youre amilitary member transitioning to civilian life we value your service andskills. Applicable MOS codes for this role include:
- Army: Physical Therapy Specialist 68F)
- Navy: Physical Therapy Technician (HMPT)
- Air Force: Physical Medicine Specialist 4J0X2
- Marines: Physical Therapy Technician (HMPT)
- Coast Guard: Health Services Technician (HS)
