Clinical Shift Schedule: Monday Friday; 8:00a 5:00p; 1 week per month

Patient Volume/Census: 8 12 inpatient; 2 4 outpatient; total 15 patients/day

Case Mix: Advanced Heart Failure patients requiring complex cardiac care
FACILITY INFORMATION
Specialty Backup Available: Multidisciplinary cardiac and surgical team
Support Staff Available: Inpatient APPs (intermittent) outpatient NP support RN MA and customer service representatives
Reason for Coverage: Supplemental locum coverage at least once per month

Credentialing Timeline: 30 days

JOB REQUIREMENTS
Licenses: Active Florida medical license
Certifications: BC/BE by the ABIM in Advanced Heart Failure/Transplant
Experience: Completion of an ACGMEaccredited heart failure fellowship; more than 5 years postfellowship preferred
Other Qualifications: Proficient in management of mechanical support devices including Impella and ECMO; excellent interpersonal and communication skills; eager to work in a highvolume multidisciplinary environment
DUTIES & RESPONSIBILITIES
Provide comprehensive care for patients with advanced heart failure in both inpatient and outpatient settings
Round on the inpatient AHF service and manage ambulatory heart failure patients
Collaborate within a multidisciplinary team to coordinate advanced cardiac care
Refer heart transplant candidates to partnered regional centers
Support management of mechanical circulatory support devices as part of an integrated program
