Codman Square Health Center Dorchester MA

Description

Primary Responsibilities:

Physicians Clinical Care Responsibilities:

  • Practice medicine consistent with established clinical standards.
  • Follow the health center clinical quality guidelines and make satisfactory efforts to achieve clinical performance targets established by the health center.
  • Responsible for teambased management of all patients on their panel including direct patient care and followup for all patients seen during regularly scheduled clinical sessions.
  • Complete documentation and billing in a timely fashion as described in the Health Center Documentation and Billing protocols.
  • Make satisfactory efforts to remain up to date on coding rules and accurately and thoroughly code all care provided.
  • Make satisfactory effort to consistently meet or exceed productivity goals and quality metric to improve health outcomes.
  • Support practice efforts to make care accessible and convenient for students individuals and families during all phases of life including infancy pregnancy and senior years.
  • Must have the ability to identify early warning signs and symptoms of risk factors associated with poverty including Adverse Childhood Effects (ACE) exposure to violence/ traumatic events physical abuse/neglect and toxic stress.
  • Must have experience in managing adult and pediatric behavioral health problems including trauma/PTSD anxiety depression ADD/ADHD SUD and cooccurring disorders; comfortable with providing brief office counseling and prescribing psychtropic medications to patients with mild to moderate behavior and mental health disorders.
  • Must have the capacity to write prescriptions for nonurgent problems acute illnesses and injuries management of chronic problems including asthma prediabetes/diabetes hypertension and HIV.
  • Must have experience in prescribing medications including controlled substances.
  • Must have experience with Adolescent sexual and reproductive health care including STI testing diagnosis treatment; STI and HIV prevention counseling/education pregnancy testing and nondirective pregnancy options counseling and prescribing or dispensing contraceptives; including emergency contraception as well as LARC (training must be completed within first six months of hire) HIV Testing HIV preexposure prophylaxis.

Physicians Administrative Responsibilities:

  • Serve as a Physician Advisor for Randolph Public School System (see attached addendum).
  • Participate in meetings training and performance improvement activities as determined by regulatory requirements and Medical Leadership.
  • Additional departmental responsibilities include but are not limited to coverage for absent Providers involvement in staff evaluations and other reasonable tasks determined by the Chief Medical Officer Medical Director Senior Administrator or Practice Manager.
  • Participate in other community activities.

Physicians Professional Responsibilities:

  • Conduct themselves in a professional manner in all formal patient staff and community interactions including being punctual to all clinical sessions and meetings.
  • Work together with medical leadership to meet the clinical obligations of the health center including clinical coverage (e.g. On Call system and coverage for Holidays primary care urgent care precepting) as determined by the CMO or delegate.
  • Must be familiar with and abide by Codman Square Health Center personnel policies procedures and guidelines clinical protocols and code of conduct.
  • Partner with health center administration Physicians will complete all compliance and licensing requirements by the deadlines outlined by Medical Leadership. Requirements include but are not limited to licensing peer review coding audits and competency assessments.
  • Partner with health center administration Physicians are required to maintain licensure and other privileges required to practice medicine in Massachusetts participate in regular continuing medical education activities; cooperate with the Codman Square Health Center staff in maintaining privileges in all hospitals and managed care plans used by the health center.
  • Participate in professional development through engaging in evaluation and peer review processes.
  • When appropriate Providers will supervise Physician Assistants and Nurse Practitioners
  • Support the formalized educational endeavors and partnerships of the health center.

Practice Transformation / Performance Improvement Responsibilities:

  • Practice in a style consistent with the Health Centers goal to provide patientcentered and team based care including sharing responsibility for care with the clinical team.
  • Participate in and lead improvement projects including transforming their personal practice to support health center initiatives.
  • Participate in teambased care including team meetings and huddles to support population management complex case management.

Physician Advisor to Randolph Public School System (RPS) Scope of Services

  • The Physician Advisor shall provide medical expertise consultation in the development and implementation of a comprehensive RPS health services program. The RPS school nurses may at any time during school business hours MondayFriday during school hours request consultation with the Physician Advisor to address specific health needs for individual students. Consultation may be requested and provided over the phone or via email. The Physician Advisor will assume responsibility for standing orders and attending quarterly health advisory board meetings at the request of the RPS Nurse Leadership Team.

    • Must be able to stand or sit for prolonged periods 50 of the time)
    • Must be able to lift 10 pounds and load onto shelves.
