Job Description
- Advocate for older and/or disabled adults and their loved ones to help make decisions related to healthcare aging and longterm care planning which will
- Reduce hospitalizations
- Improve quality of life
- Decrease the burden on the family or responsible party
- Recognize and advocate for the clients values and wishes rather than you own
- Conduct develop monitor and manage care plan and assessments to identify
- Issues and problems
- Eligibility for assistance
- Need for services
- Onboard and sign up new clients educating them on how our Care Managed Home Care (CMHC) services will help them age in place
- Review financial legal and/or medical issues and offer referrals and recommendations to specialists and other professionals
- Focusing on avoiding future problems and conserving assets
- Assist with determining the level of care needed and moving a client to a more appropriate facility or the arrangement of inhome care
- Act as liaison to responsible parties and professionals
- Facilitate our Care Managed Home Care model by:
- Manage intakes complete onsite safety assessments and complete care plans for new clients
- Provide coaching for caregivers with new clients to ensure success
- Overseeing the orientation and training to new and existing caregivers
- Complete documentation requirements accurately and timely
- Participate in marketing efforts with the goal of educating the public cultivating clients and securing new clients
