Job Description

  • Advocate for older and/or disabled adults and their loved ones to help make decisions related to healthcare aging and longterm care planning which will
    • Reduce hospitalizations
    • Improve quality of life
    • Decrease the burden on the family or responsible party
  • Recognize and advocate for the clients values and wishes rather than you own
  • Conduct develop monitor and manage care plan and assessments to identify
    • Issues and problems
    • Eligibility for assistance
    • Need for services
  • Onboard and sign up new clients educating them on how our Care Managed Home Care (CMHC) services will help them age in place
  • Review financial legal and/or medical issues and offer referrals and recommendations to specialists and other professionals
    • Focusing on avoiding future problems and conserving assets
  • Assist with determining the level of care needed and moving a client to a more appropriate facility or the arrangement of inhome care
  • Act as liaison to responsible parties and professionals
  • Facilitate our Care Managed Home Care model by:
    • Manage intakes complete onsite safety assessments and complete care plans for new clients
    • Provide coaching for caregivers with new clients to ensure success
    • Overseeing the orientation and training to new and existing caregivers
  • Complete documentation requirements accurately and timely
  • Participate in marketing efforts with the goal of educating the public cultivating clients and securing new clients
Compensation: $30.00 $35.00 per hour
