Responsibilities may include the following but are not limited to:

• Traveling to patients home and providing Best Practice nursing care
• Checking vital signs
• Documenting in a complete and confidential manner
• Evaluate the patients wellbeing reporting any needs to the Director of Clinical Nursing or emergency services
• Providing personal care and meals
• More advanced nursing such as chest tubes IVs colostomy urinary catheter management are also available
• Giving emotional support to client and families in times of crisis

Requirements:

• Valid LPN license
• Annual physical within one year
• PPD or Quantiferon within one year or chest xray within five years
• MMR labs
• COVID vaccine card with booster
• MUST HAVE PEDIATRIC EXPERIENCE
