Job Description: Family Medicine Physician Community Healthcare
Location: Los Angeles California
Position Overview:
We are seeking a dedicated and compassionate Family Medicine Physician to join our team in a community healthcare setting in Los Angeles California. This is a unique opportunity to provide comprehensive highquality care to a diverse patient population while making a meaningful impact in underserved communities. The ideal candidate will be passionate about community health skilled in managing a wide range of medical conditions and committed to reducing health disparities in the region.
Key Responsibilities:
- Provide comprehensive primary care services to patients of all ages including preventative care chronic disease management and acute illness care.
- Perform physical examinations diagnostic evaluations and minor office procedures as needed.
- Develop and implement individualized treatment plans in collaboration with patients and their families.
- Educate patients on preventive care wellness strategies and management of chronic illnesses.
- Collaborate with a multidisciplinary healthcare team including nurse practitioners physician assistants social workers and behavioral health providers.
- Refer patients to specialists or community resources as necessary to address their healthcare needs.
- Maintain accurate and detailed patient records in compliance with state and federal regulations.
- Actively participate in quality improvement initiatives to enhance the delivery of care and patient outcomes.
Requirements:
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board certification or board eligibility in Family Medicine.
- Active and unrestricted California medical license.
- DEA license with the ability to prescribe medication.
- Demonstrated commitment to serving underserved communities.
- Strong interpersonal and communication skills with a focus on patientcentered care.
Preferences:
- Bilingual proficiency (e.g. English and Spanish) a plus but not required.
- Experience working in a community health center federally qualified health center (FQHC) or similar setting.
- Knowledge of social determinants of health and their impact on patient outcomes.
