Home Health Physical Therapist (PT) FT
Serving Charleston Berkeley and Dorchester Counties.

A few musthaves for our Home Health Physical Therapists:
- Graduate of an accredited Physical Therapist Program and active PT license in South Carolina
- Minimum of 1yearof physical therapy experience ideally in home healthcare
- CPR certification
- Valid DriversLicense and Car Insurance
- Knowledge of state and federal home health regulations
- Good clinical judgement strong interpersonal skills dexterity and compassion

As a Home Health Physical Therapist heres a bigpicture view of what youll do:
- Provide homebased physical therapy to patients who have lost mobility strength and functionality due to an illness orinjury.
- Work as part of a home health team which may include an RN LPN CNA HHA OTand SLP focused on the patients plan of care and goals.
- Observe movements conduct assessments document progress and modify care plan asneeded.
- Assist patient with exercises and handson therapy to manage pain and increasemobility.
- Educate patients on their plan of care established goals exercises andselfcare.
- Coach family on the plan of care therapy goals and safely assisting withexercises.
