Are you the one were looking for

We are seeking a dynamic highly motivated Intensive Case Manager who demonstrates our values of compassion accountability responsiveness and empowerment; to our clients families and each other.

TheIntensive Case Manager is responsible for managing the case load of up to 7 dual diagnosis clients in a residential setting.

This position is aunionized fulltime temporary position. The contract is until December 31 2025. The rate of pay is $23.67 per hour and will go up to $24.38 starting April 2025. Hours per week are 40.

Key Responsibilities:

  1. Managing referrals as needed
  2. Accompanying clients to medical appointments and providing any necessary support
  3. Keeping client files up to date
  4. Helping clients work on selfidentified goals
  5. Participation of oncall rotation
  6. Managing supply orders
  7. Assisting clients with medication management meal preparation and maintaining their unit
  8. Other duties as assigned

What you will bring:

  1. Diploma in social services or equivalent
  2. CPR and NVCI certificate
  3. Minimum oneyear experience working with dual diagnosis population
  4. Experience with case management in a residential setting
  5. Exceptional written and verbal communication skills
