Job Details
The Case Manager ensures individuals are linked to mental health substance abuse and medical services as required by project initial assessment. Assists with initial referrals and providing transportation to other programs. Collaborates with other programs individuals are involved with to ensure appropriate care. Participates in oncall rotation for afterhours services. Must have a drivers license and be able to drive an agency vehicle. Position requires travel to consumers residences.
Legacy Treatment Services Integrated Care Clinic will serve residents of Atlantic County New Jersey. The project will provide outpatient mental health and substance use counseling personcentered treatment planning case management services peer support services outpatient primary care screening and monitoring services services for veterans and crisis services.
Hours:
This is a 40 hour full time position Monday Friday with some evenings required.

Education/Experience:
Bachelors degree in Psychology Social Work or related field. Must have one year experience working with psychiatric population. Masters degree preferred.
