Job Description

He aha te tranga About the role:  

As a Patient Care Plan Coordinator youll be responsible for supporting the Hato Hone St John clinical strategy at a local level. This will include working alongside Area Operations Manager Watch Operations Managers Group Operations Manager ambulance personnel community health professionals (e.g. GPs practice nurses etc) inhospital specialists and other health professionals to develop and implement new patient care plans (PCPs) for frequent callers and patients with complex needs and ensure existing PCPs remain safe effective and current.

Please note this is a fixedterm role (through until 30th June 2026 working 20 hours per week.

Key responsibilities include: 

  • Support the delivery of the Hato Hone Hato Hone St John clinical strategy by writing new PCPs for frequent callers and complex needs patients in partnership with local health professionals
  • Assist and mentor other clinical champions to write and implement new PCPs (where capacity allows)
  • Identify frequent callers and determine which patients who may benefit from a PCP
  • Ensure all existing PCPs within the region are effective safe and current by regularly reviewing plans within the PCP database and working with plan owners to ensure PCPs are updated or removed as required.
  • Work in partnership with healthcare professionals patients and their family/whanau (as appropriate) to develop effective and fitforpurpose PCPs for frequent callers and patients with complex needs
  • Escalate issues and discuss complex PCPs with key internal stakeholders as required including AOM WOM & GOMs Right Care Advisors Clinical Team Managers and the Clinical Director
  • Support the promotion and communication of the vision of PCPs and Right Care to internal stakeholders to ensure a complete shared vision is achieved
  • Contribute to audit activities and regularly review data to support the robust evaluation and ongoing improvement of the PCPs model.

Qualifications :

  • Appropriate registered health professional with current annual practicing certificate
  • Experience in building and maintaining key relationships with local health stakeholders including general practice teams
  • Extensive experience in implementing successful patient care plans
  • Experience in handling confidential and sensitive patient information
  • Has specific indepth knowledge of the challenges facing health and social care and the needs of frequent callers and patients with complex needs
  • Strong communication skills both written and oral

Please note Authority to Practice (ATP) is not required in this role although the role holder may elect to maintain their ATP.
