This job functions autonomously to deliver physical therapy treatment either through referral or directly to the consumer. It evaluates patients develops treatment plans assesses patients and monitors progress toward treatment goals. It ensures patient needs are met through access to care patient experience and quality of care metrics. It assists in program development and enhancement to meet patient and community needs.
Education
Required Bachelors degree in physical therapy.
Work Experience
Required Minimum of 2 years
Certifications
Required Current physical therapy license in the state of practice.
OR
eligibility for licensure by examination
OR
reciprocity from another state.
Basic Life Support (BLS) from the American Heart Association.
Knowledge Skills and Abilities (KSAs)

The risk level of exposure may increase depending on the essential job duties of the role.
