Social Worker Intensive Care
Job Summary
Reporting to the Clinical Services Manager Critical Care the Social Worker will be an integral part of a multi-disciplinary team.
Accountabilities:
- Responsible for the care of patients and families throughout the continuum of life dealing with such issues as bereavement including: end-of-life issues difficult family situations facilitated discharge and staff incident debriefing
- Work collaboratively with the allied health team in coordinating patient plan of care
- Represent social work in various critical care related projects
- Participate in yearly strategic planning initiatives and corporate professional development
- Provide coverage in Critical Care services and other areas of the hospital as needed
Qualifications :
- Masters degree in social work required
- Membership in OCSWSSW is required
- Previous experience in critical care would be an asset
- Three to five years experience working within a specific unit
- Previous work history of dealing with the following client types/issues:
- Assessment of the effect of physical illness on patient/family systems
- End-of-life including but not limited to unexpected death and bereavement; end-of-life communication skills
- Knowledge and application of coping strategies supporting emotional health and well-being psychosocial interventions to support the coping of patients and family members in their adjustment to illness
- Managing complex family dynamics caregiver burnout
- Knowledge of financial resources and processes to support clients
- Knowledge of medical resources supporting our clients within the community
- Knowledge of processes locating next of kin
- Ability to provide grief and bereavement therapy addictions elder abuse and child abuse support
- Elderly homeless substance abuse pregnancy loss mental health domestic violence
- Knowledgeable of community resources and ability to work with community partners
- Knowledge and experience working with relevant legislation (i.e. SDM Act POA Consent and Capacity etc.)
- Knowledge of the psychosocial impact of chronic illness on the person and the family
- Excellent communication and interpersonal skills
- May be required to travel between and work at all Osler sites
- Excellent organization skills and ability to set priorities as required
- Demonstrated time-management skills
- Demonstrated ability to work independently or part of a multi- disciplinary team
- Good critical thinking and problem-solving skills required
- Excellent attendance punctuality and work record
- Computer proficiency preferred

Additional Information :
Hours: Currently days eight hour shifts including weekends and statutory holidays (subject to change in accordance with operational requirements)
SALARY:
Minimum: $45.58
Maximum: $59.09
