Job Summary
Summary of Duties
To provide a full range of social work services to patients and their families at the Holland Orthopaedic and Arthritic Centre. This includes psycho-social assessments counselling to patients and families discharge planning crisis intervention linkage to community resources team consultation.
Qualifications/Skills
Masters Degree in Social Work
Member of the Ontario College of Social Workers and Social Service Workers
Minimum two (2) years of experience working in a Hospital setting
Exceptional communication skills required
Familiarity with issues of capacity and consent
In-depth understanding of the Sunnybrook Discharge Policy and its application
Strong negotiation and conflict resolution skills
Ability to be a team player as well as self-directed
Ability to manage multiple tasks and set priorities in a fast-paced environment
Commitment to student education
