Registered Nurse - Home Health RN

BAYADA Home Health Care is seeking a Full Time Registered Nurse (RN) to join the Fort Collins Visits team and work with adult and senior clients on a per visit basis.

Please note: current or recent experience in a Medicare Certified Home Health setting with OASIS documentation experience is strongly preferred.

As a home health nurse you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients affording them the opportunity to receive the medical care required to remain at home.

• Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
• Utilize your experience with acute and post-acute care (e.g. CHF COPD wound care IV therapy post-surgical care)
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation teaching and safety monitoring
• Perform comprehensive wound assessment care and documentation

Qualifications for a BAYADA nurse:

A current Colorado RNlicense in good standing
Graduation from an accredited and approved nursing program as indicated by school transcript or diploma

Additional Information:

• Pay: $58-63 per point based on qualifications
• Mileage reimbursement
