Empower Simcoe is currently seeking qualified candidates for the newly established role of Quality and Innovations Empowerment Facilitator. This position will support the delivery of high-quality person-directed services through collaboration data collection quality monitoring compliance activities and organizational initiatives that promote positive outcomes and service excellence.

A key responsibility of this role is supporting the organizations Self-Advocate members of the Believe in Yourself Council by coordinating meetings facilitating participation documenting feedback and ensuring the perspectives of people supported inform organizational planning quality improvement and decision-making while promoting self-advocacy inclusion and rights-based practices.

The Quality and Innovations Empowerment Facilitator coordinates the Due Process framework and supports the Independent Rights and Ethics Committee (IREC) by managing Rights Restrictions referrals documentation timelines recommendations and follow-up actions. This position ensures compliance with legislation and organizational policies while promoting ethical person-centered and least restrictive approaches that uphold the rights dignity and informed choice of people supported.

Compensation package:

  • Work up to 37.5 hours weekly
  • Minimum starting rate of pay is $32.12 per hour ($62634 annually)

Minimum requirements for this position include:

  • Minimum of three (3) years of experience in the developmental service sector with demonstrated knowledge of person-directed planning quality assurance and legislative and regulatory requirements.

Overview of duties (including but not limited to):

  • Demonstrated experience supporting quality improvement initiatives committee coordination accreditation activities and compliance processes is considered an asset.
  • Knowledge of person-centered thinking Personal Outcome Measures the Quality Assurance Measures (QAM) regulation (Ontario Regulation 299/10) Child Youth and Family Services Act 2017.
  • Experience supporting self-advocacy initiatives and working collaboratively with people supported to promote inclusion choice and meaningful participation.
  • Ability to work in a computerized environment including methods of data collection analysis and presentation.
  • Excellent written and verbal communication skills.
  • Possess strong coordination and organizational skills;
  • Current valid drivers licence and own reliable transportation to be used in the course of employment.

General Duties:

  • Participating on and support the Simcoe Resource Management Committee on behalf of Empower Simcoe.
  • Assisting with maintaining the Basic Assurances monitoring system including the collection and organization of supporting information to share with CQL.
  • Preparing reports records and recommendations to support quality monitoring and decision making.
  • Supporting the Independent Rights and Ethics Committee including the rights restriction processes tracking Behaviour Support Protocols and Rights Restrictions; scheduling meetings and completing meeting minutes.
  • Supporting the organizations Self-Advocate members of the Believe in Yourself Council by coordinating meetings facilitating participation documenting feedback and ensuring the perspectives of people supported inform organizational planning quality improvement and decision-making while promoting self-advocacy inclusion and rights-based practices.
  • Coordinating the Due Process framework and supports the Independent Rights and Ethics Committee (IREC)
  • Monitoring the complaints hotline and feedback email and entering information on the Aims database system.
  • In conjunction with the Manager providing support and guidance to Self-Advocate members of the Believe in Yourself Council.
  • Working collaboratively with relevant stakeholders to coordinate the scheduling and delivery of services requested or recommended by the Self-Advocate Committee.
  • Collaborating with Community Support Services and committee members to coordinate training needs schedules materials and attendance.
  • Assisting with maintaining current training plans records resources and completion documentation for committee members.
  • Monitoring follow-up actions and support ongoing communication between the Believe in Yourself Committee Purchased Services and Community Support Services.
  • Assisting in the completion of internal audits as directed by the Director Quality and Innovations to assist the agency in evaluating quality supports and services.
  • Organizing and completing Internal Reviews/Investigations.
  • Participating on internal and external committees as requested.
  • Performing such duties as assigned by the Director or Manager of Quality and Innovations.
