Pharmacy Student Holland Pharmacy Temporary Part-Time

Sunnybrook Health Sciences Centre Holland Orthopaedic & Arthritic Centre is seeking one dedicated and motivated Pharmacy Student enrolled in the University of Toronto International Pharmacy Graduate (IPG) Program to join our team as a Clinical Assistant to the Pharmacists.

Position Summary:
The successful candidate will work closely under the direct supervision of a clinical pharmacist preceptor to support the delivery of high-quality pharmaceutical care to our orthopedic surgery patients. This temporary part-time position offers an excellent opportunity to gain hands-on experience in a collaborative patient-centered environment.

Key Responsibilities:

• Conduct Best Possible Medication Histories (BPMH) for orthopedic surgery patients.
• Perform patient medication reviews to ensure safe and effective pharmacotherapy.
• Identify drug therapy problems and contribute to resolving medication-related issues under supervision.
• Develop and implement pharmaceutical care plans under the full supervision and guidance of a clinical pharmacist.
• Collaborate with the interdisciplinary healthcare team to optimize patient outcomes.
• Attend daily interdisciplinary rounds.
• Provide discharge medication counseling

Qualifications:

• Currently enrolled in the University of Toronto International Pharmacy Graduate (IPG) Program.
• Strong interest in hospital pharmacy practice.
• Excellent communication documentation and interpersonal skills including strong English proficiency.
• Demonstrated commitment to patient safety and quality improvement.
• Ability to work effectively within a multidisciplinary team.
• Strong attention to detail and excellent organizational skills.
• Experience working in hospital pharmacy required.
• Experience working in Canadian community pharmacy an asset.
