About the Role

We are seeking a compassionate patient-focused Bilingual Nurse Case Manager to support patients healthcare providers and clinic partners across Québec through a specialized Patient Support Program (PSP).

As a key member of the care team you will help patients navigate their treatment journey by providing education clinical support care coordination and ongoing follow-up. Working closely with physicians clinics pharmacies and internal stakeholders you will help facilitate timely access to treatment while delivering an exceptional patient experience.

This is a part-time position averaging 18.75 hours per week. Scheduled hours must fall within the programs operating hours of Monday through Friday 8:00 a.m. to 8:00 p.m. EST.

What Youll Do:

• Serve as a primary point of contact for patients enrolled in the Patient Support Program.
• Provide education regarding prescribed therapies treatment expectations medication administration and adherence strategies.
• Support patient enrollment onboarding and ongoing engagement activities.
• Coordinate care and communicate with physicians clinic staff specialty pharmacies and other healthcare professionals throughout Québec.
• Assist patients in navigating access to treatment reimbursement programs and medication support services.
• Conduct patient assessments and follow-up activities in accordance with program requirements.
• Monitor patient progress and provide ongoing support throughout the treatment journey.
• Maintain accurate and timely documentation within program systems.
• Identify and escalate clinical or operational concerns as appropriate.
• Contribute to quality improvement initiatives and program performance objectives.
• Build and maintain collaborative relationships with healthcare providers clinics and stakeholders across Québec.

Basic Qualifications:

• Active and unrestricted nursing license in Québec (OIIQ registration required) as a Registered Nurse (RN) Licensed Practical Nurse (LPN) or equivalent nursing designation recognized in Québec.
• Fluency in both French and English with excellent verbal and written communication skills in both languages.
• Strong clinical assessment patient education and care coordination skills.
• Proficiency with Microsoft Office applications and electronic documentation systems.
• Ability to work 18.75 hours per week within program operating hours of 8:00 a.m. to 8:00 p.m. EST Monday through Friday.
• Ability to travel to clinics and healthcare sites throughout Québec as required.
• Strong organizational skills attention to detail and ability to manage multiple priorities.

Preferred Qualifications:

• Experience working within a Patient Support Program (PSP).
• Previous Nurse Case Management experience.
• Experience supporting patients receiving specialty medications biologics or complex therapies.
• Knowledge of reimbursement patient access and specialty pharmacy processes.
• Experience providing patient education and clinical support in both English and French.
• Demonstrated ability to collaborate with physicians specialty clinics pharmacies and multidisciplinary healthcare teams.
• Experience supporting patients with chronic disease management and treatment adherence.
• Additional active nursing licenses in other Canadian provinces.

Travel Work Environment & Physical Requirements:

• Regular travel to Québec clinics healthcare facilities and stakeholder meetings across the province.
• Valid drivers license and access to reliable transportation required.
• Ability to work independently in a remote environment while maintaining a high level of organization and responsiveness.
• Occasional lifting and transportation of educational or program materials may be required.
