Position Summary

As a member of the IMPACT interdisciplinary team the Community Health Worker - Vocational Specialist supports individuals living with serious mental illness in pursuing employment education volunteer opportunities and other meaningful community roles that align with their recovery goals.

Duties

  • Work from a recovery-oriented trauma-informed and harm reduction framework
  • Provide individualized vocational support
  • Facilitate group programming
  • Collaborate with community partners
  • Support opportunities for social inclusion
  • Promote skill development
  • Encourage community participation

Qualifications :

  • Completion of a Community Health Worker Social Service Worker or related diploma program or an equivalent combination of education relevant experience and/or lived experience.
  • Minimum one (1) to two (2) years experience supporting individuals experiencing mental illness substance use homelessness or complex social needs.
  • Demonstrated knowledge of recovery-oriented practice harm reduction trauma-informed care and anti-oppressive approaches.
  • Experience facilitating groups workshops or psychoeducational programming.
  • Excellent interpersonal communication engagement and problem-solving skills.
  • Ability to work independently in community environments and collaboratively within a multidisciplinary team.
  • Demonstrated computer proficiency and ability to document electronically.
