Job Summary
Revolve Home Health is seeking a highly organized compassionate and proactive Care Coordinator to oversee and coordinate our home and community-based support services across Care Coordinator will be responsible for ensuring individuals receive consistent person-centred services while providing ongoing support and direction to Community Support Workers. This role serves as an important connection between clients families facilities support staff community partners and Revolve Home Health successful candidate will manage client onboarding individualized support planning staff-client matching scheduling oversight service documentation follow-ups and ongoing communication. They will also monitor service delivery to ensure supports remain aligned with each individuals goals preferences and approved service position requires someone who can work independently and manage multiple clients and staff across different communities solve problems quickly and maintain strong professional relationships. The ideal candidate is comfortable balancing administrative responsibilities with hands-on coordination and understands the importance of community inclusion client choice and quality service CandidateWe are looking for someone who is organized approachable accountable and confident working independently. Strong communication and relationship-building skills are essential as this position involves regular interaction with individuals with diverse needs families facility teams frontline staff and external experience in disability services community support social services healthcare home care case management recreation therapy or care coordination would be considered a strong is position will require occasional travel throughout Alberta for client visits facility meetings staff support community engagement and other operational needs. The successful candidate must also be comfortable using technology and learning electronic scheduling documentation and care-management systems in a remote working environment.A valid Alberta drivers licence reliable vehicle and ability to occasionally travel within Alberta are Responsibilities- Coordinate client referrals intake onboarding scheduling and ongoing services.- Develop and maintain individualized support plans and client documentation.- Coordinate schedule and provide ongoing support to Community Support Workers across Alberta.- Maintain communication with clients families facilities staff and community partners.- Monitor service delivery reports timesheets incidents and client progress.- Address scheduling staffing client and service concerns as they arise.- Assist with recruiting onboarding and supporting new staff.- Maintain accurate and confidential records and ensure documentation requirements are met.- Participate in team meetings and provide regular updates to management.- Travel within Alberta occasionally for client facility and staff & Experience- Diploma or degree in Social Work Human Services Disability Studies Health Care Administration Recreation Therapy Psychology or a related field preferred.2 years of experience in community support disability services home care case management or care coordination is an asset.- Previous experience coordinating clients and frontline support staff is strongly preferred.- Strong communication organization documentation and computer skills.- Valid drivers licence and reliable vehicle required for occasional travel within Alberta.
