Position Summary
Working as a member of the interdisciplinary Community Wellness team the Social Worker provides a diverse range of client care to ensure client engagement and to reduce or remove barriers. This care includes extended community outreach mental health support health education case management treatment support counseling community programming and advocacy for clients with acute and complex care needs. The Social Worker provides this care to individuals while promoting overall wellness and adopting a long-term approach to client care consistent with the philosophy of harm-reduction.

Qualifications

Education
Must be registered and in good standing with the Manitoba College of Social Workers
Must have a valid class 5 Drivers License and access to a reliable vehicle

Skills Expertise and Experience
A minimum of five (5) years of experience working as a Social Worker
A minimum of three (3) years of experience working in the areas of addictions mental health crisis intervention and cognitive behavioral therapy.
Experience working with a vulnerable population.

Proven knowledge and ability in relationship-building critical thinking facilitation decision-making problem-solving and conflict-resolution skills

Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes

Experience working with service-specific computer programs such as EMR eChart and Accuro
