Health and Wellness Coach

Join our passionate team at the MartinsvilleHenry County Coalition for Health and Wellness now known as Connect Health Wellness as a Health and Wellness Coach. Since our inception in April 2005 with a foundational grant from the Harvest Foundation we have dedicated ourselves to providing comprehensive medical dental and health services especially to the uninsured and underserved in the MartinsvilleHenry County area. As part of our mission to increase access reduce risks and improve community health we embrace core values of Respect Excellence Compassion Integrity Teamwork and Equity. Tasks

• Provide individualized health coaching to clients to help them set and achieve wellness goals
• Develop personalized wellness plans based on clients health history lifestyle and goals
• Educate clients on healthy lifestyle choices nutrition exercise stress management and other wellness topics
• Monitor client progress and provide ongoing support and motivation to help them stay on track
• Collaborate with other healthcare professionals to ensure comprehensive care for clients

Requirements

• Bachelors degree in Health Promotion Public Health Nutrition Exercise Science or related field
• Certification as a Health Coach or Wellness Coach preferred
• Minimum of 2 years experience in health coaching or wellness counseling
• Strong communication and interpersonal skills
• Ability to work collaboratively with a diverse team and client population
