Kace Premier Medical Talent is recruiting a Physical Therapist Assistant for a skilled nursing rehabilitation opportunity in Hayward California. Work under a Physical Therapists direction to help patients improve mobility and functional independence.

Responsibilities:
Implement interventions within the Physical Therapists established plan of care.
Provide therapeutic exercise gait training balance activities and functional mobility training.
Monitor patient responses and report progress or concerns to the supervising therapist.
Reinforce patient and caregiver education on safe movement and prescribed exercises.
Complete accurate treatment documentation.
Collaborate with the rehabilitation and nursing teams.
Follow applicable supervision requirements and professional scope of practice.

Qualifications:
Graduate of an accredited physical therapist assistant program.
Current California PTA license or eligibility to obtain licensure before starting.
Strong communication teamwork and documentation skills.
Ability to provide individualized care under PT supervision.
