Nurse (RN)

As a Registered Nurse you will provide skilled nursing care in a collaborative outpatient environment while helping patients achieve optimal health outcomes.

Key Responsibilities
  • Deliver skilled nursing services under the direction of physicians and advanced practice providers
  • Assess patient health needs and analyze clinical data
  • Develop implement and evaluate individualized plans of care
  • Monitor patient progress and modify care plans as appropriate
  • Collaborate with providers and interdisciplinary healthcare teams
  • Educate patients and families regarding health conditions treatments and preventive care
  • Maintain a safe therapeutic and patient-focused environment
Registered Nurse Care Coordinator (RNCC)

As a Registered Nurse Care Coordinator you will play a critical role in supporting patients across the continuum of care with a focus on coordination communication and improving health outcomes.

Key Responsibilities
  • Partner with providers and clinic teams to deliver evidence-based patient-centered care
  • Support patients in managing care plans and achieving health goals
  • Coordinate care for high-risk and medically complex patients
  • Assist patients with chronic conditions disabilities and disease-specific care management needs
  • Facilitate smooth care transitions including hospital discharge follow-up and transitions between care settings
  • Serve as a liaison among patients families healthcare providers and community resources
  • Promote comprehensive whole-person care through proactive communication and care coordination

Qualifications
Minimum Qualifications
Required for Both Positions
  • Associate Degree in Nursing from a CCNE or ACEN accredited nursing program
  • Current Ohio Registered Nurse license
  • Current CPR/BLS certification
  • Experience in clinic physician office urgent care or related healthcare settings
  • Knowledge of triage procedures and patient care standards
  • Strong communication and patient education skills
  • Ability to maintain quality safety and infection control standards
  • Excellent interpersonal organizational and critical-thinking skills
  • Ability to maintain confidentiality and handle sensitive situations with professionalism and diplomacy
