KEY RESPONSIBILITIES
- Provide comprehensive hospice nursing care to terminally ill patients in home and facility settings
- Conduct initial and ongoing assessments; manage pain and symptom control
- Educate patients and family caregivers on disease progression and end-of-life care
- Coordinate with hospice interdisciplinary team including social work chaplaincy and aide services
- Document clinical assessments medications and visit notes in EMR
REQUIREMENTS
Must-Have
- Active Texas RN licence or compact licence eligibility
- BLS certification - current
- Minimum 1 year hospice home health or palliative care RN experience
- Valid drivers licence and reliable transportation
Preferred / Nice-to-Have
- CHPN certification
- Case management experience
