Job Overview

We are seeking a skilled and compassionate Physical Therapist (PT) for a travel assignment in a skilled nursing and rehabilitation setting. The ideal candidate will evaluate patients develop individualized treatment plans and provide therapeutic interventions to improve mobility strength balance and overall functional independence.

This role is ideal for therapists who are passionate about helping patients achieve their rehabilitation goals and maximize quality of life.

Key Responsibilities

Evaluate patients and develop individualized treatment plans
Provide evidence-based physical therapy interventions
Assess patient progress and modify treatment plans as needed
Improve patient mobility balance strength endurance and functional abilities
Educate patients families and caregivers on therapy goals and home exercise programs
Maintain accurate and timely clinical documentation
Collaborate with physicians nurses and rehabilitation team members
Ensure compliance with facility policies and regulatory standards

Start Date: 06/01/2026
End Date: 08/31/2026

Shift: Day Shift
Shift Time: 08:00 16:30

Schedule: As assigned by facility
Guaranteed Hours: 35 Hours per Week
Contract Length: Approximately 14 Weeks

Requirements

Active Physical Therapist (PT) license required
Graduate of an accredited Physical Therapy program
Skilled nursing rehabilitation or post-acute care experience preferred
Strong assessment treatment planning and patient education skills
Excellent communication and interdisciplinary collaboration abilities
Must comply with facility onboarding and compliance requirements
Must bring state or federally issued identification and wear a photo badge at all times
