Job Overview

We are seeking a skilled and compassionate Physical Therapist (PT) for a travel contract assignment in a skilled nursing and rehabilitation setting. The Physical Therapist will evaluate patients develop individualized treatment plans and provide therapeutic interventions designed to improve mobility strength function and overall quality of life.

Key Responsibilities

Evaluate patients and develop individualized treatment plans
Implement physical therapy interventions to improve mobility and function
Monitor patient progress and modify treatment plans as needed
Educate patients families and caregivers on treatment goals and home exercise programs
Collaborate with interdisciplinary healthcare teams
Maintain accurate and timely clinical documentation
Ensure compliance with professional and regulatory standards
Promote patient safety and quality outcomes throughout the rehabilitation process

Start Date: 06/22/2026

End Date: 09/19/2026

Shift: Day Shift

Shift Time: 08:00 16:30

Guaranteed Hours: 35 Hours per Week


Requirements

Active Physical Therapist (PT) license required
Graduate of an accredited Physical Therapy program
Previous skilled nursing rehabilitation or long-term care experience preferred
Strong assessment treatment planning and communication skills
Ability to work independently and as part of a multidisciplinary team
Current CPR/BLS certification preferred
Must comply with all onboarding and credentialing requirements
Must bring state or federally issued identification and wear a photo badge at all times during assignment
