Job Overview

We are seeking a Physical Therapist (PT) for a contract assignment in a rehabilitation setting. The PT will evaluate patients develop individualized treatment plans and implement therapeutic interventions to improve mobility strength function and quality of life.

Key Responsibilities

Perform patient evaluations and functional assessments
Develop and implement individualized treatment plans
Provide therapeutic exercises and manual therapy interventions
Monitor and document patient progress
Educate patients and caregivers on rehabilitation goals and home programs
Collaborate with interdisciplinary healthcare teams
Ensure compliance with clinical standards and regulations
Maintain accurate and timely documentation

Start Date: 06/15/2026
End Date: 09/14/2026

Shift: Day Shift
Shift Time: 08:00 AM 04:30 PM
Schedule: Monday Friday
Contract Length: 13 Weeks
Guaranteed Hours: 35 per week

Requirements

Active Physical Therapist license (state required)
Graduate of accredited Physical Therapy program
CPR/BLS certification preferred
Experience in rehab SNF outpatient or acute care settings preferred
Strong clinical assessment and treatment planning skills
Excellent communication and documentation abilities
Valid state/federal photo ID required onsite at all times
Compliance with onboarding and health requirements
