Job Overview

We are seeking a skilled and motivated Physical Therapist (PT) for a travel assignment in a rehabilitation and skilled nursing setting. The ideal candidate will evaluate patient needs develop personalized treatment plans and provide evidence-based therapy interventions to improve mobility strength and overall functional independence.

This position is ideal for therapists who enjoy helping patients recover from illness injury or surgery while working in a collaborative healthcare environment.


Key Responsibilities

Evaluate patients and establish individualized treatment plans
Deliver physical therapy treatments to improve mobility and functional outcomes
Monitor patient progress and adjust treatment plans as appropriate
Educate patients and families regarding therapy goals and home exercise programs
Collaborate with physicians nurses and interdisciplinary healthcare teams
Maintain accurate and timely clinical documentation
Ensure compliance with facility policies safety standards and regulatory requirements
Promote patient-centered care and rehabilitation success


Assignment Details

Start Date: 06/01/2026
End Date: 08/31/2026

Shift: Day Shift
Shift Time: 08:00 16:30

Schedule: As Assigned
Guaranteed Hours: 36 Hours per Week
Contract Length: Approximately 14 Weeks


Requirements

Active Physical Therapist (PT) license required
Graduation from an accredited Physical Therapy program required
Previous rehabilitation or skilled nursing experience preferred
Strong assessment treatment planning and patient education skills
Excellent communication and teamwork abilities
Ability to work independently and manage a diverse patient caseload
Must comply with facility onboarding and compliance requirements
Must bring state or federally issued identification and wear a photo badge at all times
