Job Overview

We are seeking a Physical Therapist (PT) for a contract assignment. The Physical Therapist will evaluate develop and implement individualized treatment plans to help patients improve mobility reduce pain restore function and achieve optimal physical performance.

Key Responsibilities

Evaluate patients and develop individualized treatment plans
Provide therapeutic interventions to improve strength balance mobility and function
Monitor patient progress and modify treatment plans as needed
Educate patients and caregivers on treatment goals and home exercise programs
Maintain accurate and timely documentation of patient care
Collaborate with interdisciplinary healthcare teams to optimize outcomes
Ensure compliance with regulatory and professional practice standards
Promote patient safety and quality care throughout the rehabilitation process

Shift Time: 08:00 AM 04:30 PM
Schedule: Per contract schedule
Contract Length: 14 Weeks
Guaranteed Hours: 35 per week

Requirements

Active Physical Therapist license (state required)
Graduate of an accredited Physical Therapy program
Previous experience in rehabilitation skilled nursing outpatient or acute care settings preferred
Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Ability to work independently and collaboratively within a multidisciplinary team
Valid state/federal photo ID required on-site at all times
Compliance with onboarding and health requirements
