KEY RESPONSIBILITIES
- Evaluate patients and develop individualized physical therapy treatment plans in a rehabilitation setting
- Provide direct therapeutic interventions targeting mobility strength balance pain management and functional independence
- Supervise and direct COTA/PTA staff in the delivery of treatment as applicable
- Document evaluations daily progress notes and discharge summaries accurately in the EMR
- Collaborate with the interdisciplinary team including OT SLP nursing and physicians
- Participate in care plan meetings discharge planning and family education
REQUIREMENTS
Must-Have
- Active California PT licence - current and in good standing
- CPR/BLS certification - current
- Minimum 1 year of PT experience in a rehabilitation post-acute or SNF setting
- Proficiency with electronic medical records documentation
Preferred / Nice-to-Have
- Experience with geriatric and post-surgical rehabilitation populations
- Familiarity with Medicare Part A/B documentation requirements
