Job Overview

We are seeking a dedicated Occupational Therapist (OT) for a contract assignment in a skilled nursing and rehabilitation setting. The Occupational Therapist will evaluate develop and implement individualized treatment plans designed to help patients improve daily living skills functional mobility and overall independence.

This position includes coverage at the primary facility with floating responsibilities to an additional nearby rehabilitation center as needed.


Key Responsibilities

Evaluate patients and develop individualized occupational therapy treatment plans

Implement therapeutic interventions to improve functional abilities and independence

Assess patient progress and modify treatment plans as necessary

Educate patients families and caregivers on treatment goals and home exercise programs

Collaborate with physicians nurses rehabilitation staff and interdisciplinary care teams

Maintain accurate and timely clinical documentation

Ensure compliance with facility policies professional standards and regulatory requirements

Promote patient safety and quality outcomes throughout the rehabilitation process

Start Date: 08/03/2026

End Date: 11/01/2026

Shift: Day Shift

Shift Time: 08:00 16:30

Schedule: Monday Friday

Guaranteed Hours: 35 Hours per Week

Contract Length: Approximately 13 14 Weeks


Requirements

Active Occupational Therapist (OT) license required

Graduate of an accredited Occupational Therapy program

Previous skilled nursing rehabilitation or long-term care experience preferred

Strong clinical assessment and treatment planning skills

Excellent communication and interdisciplinary collaboration abilities

Ability to work independently and manage a diverse patient caseload

Must comply with facility onboarding requirements

Must bring state or federally issued identification and wear a photo badge at all times
