Job Overview

We are seeking a Local Licensed Practical Nurse (LPN) for a contract assignment. The LPN will provide direct patient care administer treatments monitor patient conditions and support the interdisciplinary healthcare team in delivering high-quality care.

Key Responsibilities

Provide direct nursing care to assigned patients
Administer medications and treatments as prescribed
Monitor assess and document patient conditions
Assist with care planning and implementation
Communicate effectively with physicians nurses and healthcare team members
Maintain accurate and timely clinical documentation

Shift: Day Shift
Shift Time: 07:00 AM 07:30 PM

Contract Length: 13 Weeks
Guaranteed Hours: 32 per week

Requirements

Active Licensed Practical Nurse (LPN) license
Previous long-term care or skilled nursing experience preferred
PCC proficiency required
Must be flexible with scheduling including a combination of 12-hour and 8-hour shifts as needed
Strong clinical communication and documentation skills
Ability to work effectively within a multidisciplinary healthcare team
Compliance with onboarding and facility requirements
Must maintain all required credentials and certifications
Valid state/federal photo ID required onsite at all times
