Job Overview

We are seeking a dedicated Licensed Practical Nurse (LPN) for a travel assignment in a long-term care/skilled nursing facility. This role focuses on providing direct patient care supporting nursing operations and ensuring high-quality clinical outcomes for residents.

The LPN will be responsible for administering medications monitoring patient conditions and assisting with care planning in collaboration with the registered nursing team.

Key Responsibilities

Provide direct patient care under RN supervision
Administer medications and treatments as prescribed
Monitor and document patient conditions
Assist with care planning and patient assessments
Support residents with daily living and clinical needs
Maintain accurate and timely medical documentation
Follow facility policies and patient safety standards

Requirements

Active Licensed Practical Nurse (LPN) license
Ability to work night shifts and rotating schedule patterns
