We are seeking a Licensed Practical Nurse (LPN) to provide high-quality patient care in a long-term care setting. The LPN will be responsible for delivering direct nursing care during evening shifts monitoring patient conditions administering medications and supporting the interdisciplinary care team.

This role requires strong clinical judgment excellent communication skills and the ability to work effectively in a fast-paced environment.

• Provide direct nursing care to patients
• Administer medications and treatments as prescribed
• Monitor and document patient status and changes
• Support ADL assistance and patient comfort care
• Communicate effectively with the healthcare team
• Maintain accurate and timely clinical documentation
• Follow facility policies and nursing standards
• Ensure patient safety throughout assigned shift

• Active LPN/LVN license (required)
• Completion of an accredited nursing program
• Strong clinical and documentation skills
• Ability to work evening shifts (15:00 23:30)
• Prior long-term care or skilled nursing experience preferred
• Strong teamwork and communication abilities

• Pay Rate: $36.33 $42.38/hr
