Job Overview

We are seeking a Licensed Practical Nurse (LPN) for a Travel contract assignment in a skilled nursing and long-term care setting. The LPN will provide direct patient care administer medications monitor resident conditions and support the interdisciplinary team in delivering safe high-quality care.

This role is suited for Travel LPNs who are available for full-day shifts and prefer consistent long-term care experience in a structured clinical environment.

Key Responsibilities

Administer prescribed medications and treatments
Monitor assess and document patient conditions and changes
Provide direct nursing care according to individualized care plans
Assist with admissions discharges and resident assessments
Communicate patient updates to supervising nurses and healthcare team
Maintain accurate and timely clinical documentation
Support infection prevention and safety protocols
Collaborate with interdisciplinary care teams

Guaranteed Hours: 36 Hours per Week
Contract Length: Approximately 13 Weeks

Requirements

Active Licensed Practical Nurse (LPN) license required
Experience in long-term care or skilled nursing preferred
Strong clinical and communication skills
Ability to work full 12 hour shifts
Must comply with onboarding and facility requirements
Must bring state or federally issued identification and wear a photo badge at all times
COVID-19 testing or vaccination compliance required per facility policy
