Job Overview

We are seeking a Licensed Practical Nurse (LPN) for a night shift contract assignment in a skilled nursing and long-term care setting. The LPN will provide direct patient care administer medications monitor resident conditions and support the nursing team in delivering safe and effective care.

This role is ideal for LPNs who are confident working overnight shifts and managing patient care independently.

Key Responsibilities

Administer medications and treatments as prescribed
Monitor assess and document patient conditions and changes
Provide direct nursing care according to care plans
Assist with admissions discharges and resident evaluations
Communicate patient updates to supervising RN or care team
Maintain accurate and timely clinical documentation
Support infection control and safety protocols
Collaborate with interdisciplinary healthcare staff

Guaranteed Hours: 36 Hours per Week

Requirements

Active Licensed Practical Nurse (LPN) license required
Experience in long-term care or skilled nursing preferred
Strong clinical assessment and documentation skills
Ability to work night shifts independently
Must comply with onboarding and facility requirements
Must bring state or federally issued identification and wear a photo badge at all times
COVID-19 vaccination or approved exemption may be required per policy
